Please read each league description and understand which one to select for your child or children. No Refunds will be permitted .

    Little Kickers: Pre-K/K COED (U6) - Must be 5 years old by 7-31-2024 - Cost $50.00 and plays on Tues/Thurs.

    Passers: 1st and 2nd Grade COED (U8) - Cost $50.00 and plays on Mon/Weds.

    Wings: 3rd and 4th Grade (U10) - Cost $65.00 and plays on Tues/Thurs.

    Strikers: 5th and 6th Grade (U12) - Cost $65.00 and plays on Mon/Weds.

    Kickers: 7th and 8th Grade (U15) - Cost $65.00 and plays on Tues/Thurs.

    VOLUNTEERS ARE NEEDED - IF NOONE VOLUNTEERS THEN THAT AGE GROUP MAY NOT PLAY

    The organization is always in need of volunteers, coaches, referees, etc. to assist in having a successful season. Please consider on of these opportunities to give back to your community! Again, if an age group does not have any volunteer coaches, that age group may not get to play!

    WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

    ASSUMPTION OF RISK/WAIVER OF LIABILITY/INDEMNIFICATION AGREEMENT

    In consideration of being allowed to participate on behalf of PCYSL (Putnam County Youth Soccer League West) athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

    1. Participation includes possible exposure to an illness from infectious diseases including but not limited to MRSA, influenza and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist;

    2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation;

    3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately;

    4. I, for myself and behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS PCYSL their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

    I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY INITIALING IT, AND INITIAL IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities.

    PARENT'S CODE OF ETHICS

    As parents, we will abide by the rules of the game and treat the referees with respect, no matter what he/she calls. Help maintain a climate of enjoyment among spectators by discouraging negative remarks and/or harassment of players, referees, coaches, and other spectators. Exhibit exemplary conduct at games as team discipline reflects the parents’ attitude 100%.     LINDSAY'S LAW

    Lindsay’s Law went into effect in 2017 to inform and educate students and youth athletes participating in or desiring to participate in an athletic activity, their parents, and their coaches about the nature and warning signs of sudden cardiac arrest. It is a requirement that all parents and student athletes under age 19, watch the video and review the handout. They can be found at:
