Soccer Program & Registration

The PCYSL Soccer registration form will not be sent home this year as we’ve moved to an online version only.  The forms must be filled out and submitted online to Ottoville Park Board by May 31st along with the appropriate entry fee.  There will be a $50 late fee for registrations received after May 31, 2024 and NO registrations will be accepted after June 3, 2024.  Please follow the Registration Form links provided below to complete the registration. If there are any questions, please contact one of the Soccer Board members.  Reminder to also watch the Lindsay’s Law video as you will be required to initial on the forms that you have watched and understood the video.  In addition, COVID-19 Waiver forms need to be signed and returned with registration.
